CFIA facility questionnaire for the import of plant-based feed ingredients of concern subject to mitigation measures after import to Canada

This facility questionnaire must be completed by any facility importing plant-based feed ingredients of concern from a country listed in appendix 3 of 'Import Requirements for Plant-based Feed Ingredients Imported for Use In Livestock Feed' that will be subjected to mitigation measures after import to Canada. The harmonized system (HS) codes of the specific products are found in appendix 2 of 'Import Requirements for Plant-based Feed Ingredients Imported for Use In Livestock Feed'. The purpose of the questionnaire is to provide a facility a means by which to demonstrate that the risk of transmission of African swine fever (ASF) via contamination of these products will be mitigated after arrival in Canada.

The Canadian Food Inspection Agency (CFIA) may, at any time, perform an on-site visit to verify that the information provided within this questionnaire is complete and accurate as presented. Completed questionnaires are valid for 1 year.

Note: the importation of livestock feeds may be subject to further import conditions administered and enforced by the CFIA. Depending on the origin and type of product being imported, an additional CFIA plant health import permit may be required, or there may be restrictions in place that would prohibit the importation of the plant or plant product entirely, regardless of end use.

It is the responsibility of all importers to be aware of the applicable import requirements for any products they wish to bring into Canada and to ensure that each shipment meets those requirements when it arrives at the Canadian border. The Automated Import Reference System (AIRS) shows the import requirements for Canadian Food Inspection Agency (CFIA) regulated commodities.

Facilities acting as third parties

As described in the document Import Requirements for Plant-based Feed Ingredients Imported for Use In Livestock Feed, the permit application and questionnaire for raw or unprocessed grains and oilseeds must be completed by the facility that will be processing the product once it arrives in Canada or that will be contracting with a third party to process the product once it arrives in Canada. An import permit will not be issued to a broker in this case. Where a third party processor is engaged to process the product, the importer remains responsible for ensuring that all applicable conditions are met, including those that are to be met by the third party processor.
