Guidelines to Completing the Fertilizer or Supplement Registration Application Form

This document was created to assist registrants/applicants to thoroughly and accurately complete the Fertilizer or Supplement Registration Application (CFIA/ACIA 3778) (FSRA) form. As a legal document, the form must be completed in its entirety. The Fertilizer Safety Section reserves the right to return a Fertilizer or Supplement Registration Application to the applicant if the form is deemed incomplete, or if the information provided does not exactly match the information stated on the accompanying label.

Product Type (mandatory)

Please select the corresponding product type for registration. To help determine if your product requires registration please refer to Registration Triggers for Fertilizers and Supplements.

Farm Fertilizer Registration

Select the Farm Fertilizer Registration box when the product requires registration as a farm fertilizer.

The Fertilizers Regulations define a "farm fertilizer" as any fertilizer other than a specialty fertilizer, and a "a specialty fertilizer" as a fertilizer recommended for use only on household plants, urban gardens, lawns or golf courses or in nurseries or greenhouses.  

A fertilizer is any substance or mixture of substances containing nitrogen, phosphorus, potassium or other plant food, manufactured, sold or represented for use as a plant nutrient. 

Fertilizer-Pesticide Registration

Select the Fertilizer-Pesticide Registration box when the product requires registration as a fertilizer-pesticide.

If pesticide or disease resistance claims are made on the label, and the pesticide is not registered under  the Pest Control Products Act, approval and/or registration from the Pest Management Regulatory Agency (PMRA) may be required.

Micronutrient Registration

Select the Micronutrient Registration box when the product requires registration as a micronutrient.  A micronutrient is a fertilizer that contains the lesser plant nutrients: Boron (B), Chloride (Cl), Copper (Cu), Iron (Fe), Molybdenum (Mo), Manganese (Mn), or Zinc (Zn), and that does not contain Nitrogen (N), Phosphorus (P) or Potassium (K). Micronutrient fertilizers can also contain guarantees for Ca, Mg, and/or S but these blends require registration unless intended for specialty markets only.

Supplement Registration

Select the Supplement Registration box when the product requires registration as a supplement. Supplement means any substance or mixture of substances, other than a fertilizer, that is manufactured, sold or represented for use in the improvement of the physical condition of soils or to aid plant growth or crop yields. 

Registerable supplements can be chemical in nature (e.g. plant growth regulators, wetting agents) or microbial in nature (contain viable microorganisms).

Brand Name (if applicable)

Enter here the name of the Brand (must be identical to the brand name found on the marketplace label).

Brand refers to any distinctive mark or trade name, other than the name and grade required by the Fertilizers Regulations, that is applied by the manufacturer, registrant or vendor to distinguish the fertilizer or supplement from any other product.

If the brand name is bilingual on the label (appears differently in French and English), both the French and English wording must be included on the registration form.

Name and/or Grade (mandatory)

Enter here the name of the product (must be identical to the product name found on the marketplace label).

When applicable the product name must contain the following:

  • the grade (for registerable products represented to contain any of the major nutrients N, P or K, e.g. 10-0-20). In the case of mixed fertilizers, the numerals must be in whole numbers only;
  • percentage and name of the guaranteed pesticide active ingredient.

If the product name is bilingual on the label, both the French and English wording of the name must appear on the FSRA form.

Constituent Materials (mandatory)

Enter here the complete name and source of each input material used in the production of the final product.
Enter here the composition of the Final Product.

You may include additional information in your submission package, and connect this information to the FSRA form by adding the following suggested wording "refer to submitted documents" for a new registration and/or "refer to product file [add the product registration number]" for the submission for re-registration, minor amendment, major amendment, or me-too, in the Constituent Materials Section of the FSRA form.

Other Qualities (mandatory)

Enter here the physical characteristics of the product (e.g. liquid, granular, pH, odor, size guide number (SGN) for blending products, etc.).

Enter here a description of the manufacturing process of the final product, accompanied by a flow chart diagram.

Enter here all Quality Assurance / Quality Control (QA/QC) procedures used during the manufacture of the product to ensure consistency in production, and purity of the final product.

You may include additional information in your submission package, and connect this information to the FSRA form by adding the following suggested wording "refer to submitted documents" for a new registration and/or "refer to product file [add the product registration number]" for the submission for re-registration, minor amendment, major amendment, or me-too, in the Other Qualities Section of the FSRA form.

Refer to the Guide to Submitting Applications for Registration Under the Fertilizers Act for more information in regards to requirements on physical characteristics of the product, the manufacturing process of the final product, and the Quality Assurance/Quality Control (QA/QC) procedures.

Guaranteed Analysis (mandatory)

Enter here (when applicable) the percentage of each plant nutrient guarantee, and the percentage of other related guarantees (e.g. EDTA).

Enter here (when applicable) the complete names of pesticide/active ingredient(s).

Enter here (when applicable) the complete name and concentration of each supplement active ingredient. (Note: Chemical supplement active ingredients are to be expressed as w/w percentage, and biological supplement active ingredient are to be expressed, for example, in CFU or spores per gram).

Name and Address of Manufacturer (mandatory)

Enter here the manufacturer's name and full postal address, including the country. A manufacturer is the company or individual who produces the final product. Make sure to include all manufacturing sites. The list of manufacturing sites can be provided on a separate sheet of paper and be referenced on the FSRA form by adding statement such as "refer to attached list of manufacturing sites".

If the name and address of the manufacturer appears on the product label, the information on the FSRA form must be exactly the same as that on the product label.

Name and Address of Applicant (Registrant) (mandatory)

Enter here the applicant's name and full postal address. An applicant is a person who makes an application for registration. A registrant is a person who has been issued a certificate of registration under the Fertilizers Act and Regulations. If the name and address of the applicant (or registrant) is the same as the manufacturer's name and address, it is acceptable to check the box "same as manufacturer".

If the name and address of the applicant (registrant) appears on the product label, the information on the FSRA form must be exactly the same as that on the product label.

Which Name Will Appear on the Label? (mandatory)

Check either the "registrant" or "manufacturer" box to reflect the information that appears on the product label. The respective information must appear on the product label exactly the same as it appears on the FSRA form.

Signature of Authorized Representative (mandatory)

Enter here a clearly printed name and the accompanying signature (include the date of signing) of an authorized representative. This is necessary in order to proceed with the registration of a product.

To ensure that all applications are legitimate and to protect confidential business information, the applicant (registrant) - a corporation, partnership, sole proprietorship or individual - must provide the Fertilizer Safety Section with a letter authorizing individual(s) to sign fertilizer or supplement registration application forms on behalf of the company.  For more information on signing authority, please refer to trade memorandum T-4-95: Signing authority, Delegated Representatives and Canadian Agents.

Include with the submission the appropriate documentation regarding the designation of the signing authority, unless previously provided and therefore already on file with CFIA.

Name, Address and Signature of the Resident Agent if the Applicant is Not a Resident in Canada

Enter here (if applicable) a clearly printed name and complete address of the Resident Agent, and the accompanying signature.

It is not mandatory that the registrant/applicant resides in Canada. However, as stated in the Fertilizers Regulations, if the applicant does not reside in Canada, the fertilizer or supplement is not eligible for registration unless the FSRA form is signed by an agent of the applicant who is a permanent resident in Canada (Resident Agent) and to whom any legal notice or correspondence may be sent.

Include with the submission the completely and accurately filled Declaration of Resident Canadian Agent (CFIA/ACIA 1194) form (Schedule IV), unless previously provided and therefore already on file with CFIA.
