Beef, Pork and Poultry:
The certificate shown in Annex A (Albanian version of CFIA/ACIA 1454) must be completed along with form CFIA/ACIA 1454.

The serial number which is on Form CFIA/ACIA has to be typed on Annex A. The Ottawa copy of the CFIA/ACIA 1454 is then to be forwarded to Ottawa stapled with a copy of Annex A. Special packaging or marking requirements
